Identifying patients at risk for re-hospitalization and providing in-home care can help to prevent hospitalization. High risk patients that receive home health care are significantly less likely to be readmitted to the hospital.
A report from the Robert Wood Foundation in February 2013 stated that one in five elderly patients are back in the hospital within 30 days of leaving. Many of these patients could prevent hospitalization with more assistance and diligence in regard to their after hospital care.
