This honor reflects the dedication of our nurses, therapists, and support staff who strive every day to deliver high-quality, patient-centered care that makes a meaningful difference in the lives of those we serve.
CMS established the Home Health Compare (HHC) website on Medicare.gov as a key tool for consumers to use when choosing a home health care provider. It is designed to be an easy-to-access, convenient source of authoritative information on provider quality. CMS has begun adding new tools to HHC in order to make the information there easier to use – “star ratings” that will summarize some of the current measures of health care provider performance that the site already offers. The star ratings are an additional tool to support consumers’ health care decision-making.
Home Care Elite honors the top-performing agencies as measured by five categories: quality of care (outcomes), best practice (process measure) implementation, quality improvement and consistency, patient experience (HHCAHPS) and financial performance.
HomeCare Elite was compiled using publicly available data from Home Health Compare and the Centers for Medicare & Medicaid Services (CMS) Cost Reports. The HomeCare Elite also recognizes the Top 100 and Top 500 providers nationwide. Out of 9,718 agencies considered, 2,431 are HomeCare Elite. This year marks the tenth anniversary of the HomeCare Elite. Over the course of this program, only 131 agencies achieved HomeCare Elite recognition 10 years running.

” The Elite team has and continues to be great in helping me recover from my surgeries. The physical and occupational therapists helped me with walking, how to remove hazards in my home, etc. I continue to receive services 3xs a week from a wound nurse because my bandage needs to be changed frequently.
